Well, neither you'll be transferred in the body of your 15-year-old self nor you'll take up her place. You BOTH will be present at the same place, same time.

What about you at this time?

Well, since the present you and the past you will be in the same timeline, the present you will of course disappear. Your existence will vanish. Be prepared with the reason for your disappearance once you get back to the present though. And since if you vanish for long, you'll never come back, meaning you'll lose your chance AND your life on this earth. And since the present you doesn't exist, the presence of a past you is pointless. Meaning, you will die. Also, be extremely careful that nobody catches both versions of yourselves together.
