"The Ultimate Choice" essentially is you choosing (as the subtitle says) whether you're here (on earth) to live or to die. 

If you're here to live and enjoy what you can while you're breathing you're seen as "living." If you're here to die and just waiting for the inevitable death, you're merely "existing."

Only you can choose what you're here for.
