Yup! Goat Mom has been attacked too! Even the Ruins aren't safe from a being who can RELOAD, SAVE and RESET.

Because of the deaths of her children still impacting her life, and a thorny golden flower grew out of her left eye socket (it's also a reference to Scootaloo Loves Sans uwu). She is now half-blind, the left eye socket is always bleeding (hence all the literal blood on her hands) due to the flower literally pushing her left eye out of the socket, and a little more... let's say... willing to defend herself  because of what Papyrus did to her. Yep! Papyrus did that! But you'll learn why another time.

The crown carved into her chest and the torn ear are both some more symbolism. She "killed" her responsibilities as Queen when she ran away, much like Papyrus has killed her across multiple timelines, but even though she left, her longing for her old responsibilities and role as the Queen and her everlasting love for Asgore still leave a hole in her heart that she desperately wants to fill, much like how a wound heals over time. The torn ear represents how she refused to listen to Asgore in their argument, even though he was being sensible, and the physical pain it causes her almost matches the emotional pain and guilt she feels for not staying with him to help him through the grief of losing their children.
