Here's the deal. Tsukishima was forced dress up as a girl for a school festival. Since no one else in the school knew he was trans except for a select few, people were complimenting him, saying he looked pretty in the make up, and actually looked like a girl. He absolutely hated it. Of course the third years comforted him as much as he could, and Yamaguchi offered to try and get him out of as many shifts as possible so people didn't see him. On one of his water breaks, he heard some boys talking about him behind his back. Usually, he'd try and stop them, until he overheard some words exchanged. "Yeah, he really looks like a girl! Maybe we should call him 'Tsukishima-chan' instead". He didn't know why, or how, but with no rhyme or reason he raced to the boys bathrooms, locking the door to a stall, and tightening his binder more than he should have, before walking out of the stall as if nothing happened.

Later that day when he was home, he still hadn't loosened it. That's around the time you came for your daily visit (either for homework or just to chat), and as you went to his room, you heard quiet sobs.
