His friend has two arms not one, bit in the photo his right arm was missing. It was gone. As if he didn't have a right arm.

His friend was slowly disappearing and the moment he fully disappears, Kusanagi will come.

This was the risk of having Sasuke transform into his kid self. He could do it for a few minutes before Kusanagi finds out, but anything more than that will result in pain and torture.
