1.) When you turn in a form write whatever you felt during that time.

2.) Please don't put names of bystanders, teachers, classmates, bullies, etc (or fake names.) If you choose to put your own name that's fine.

3.) Say whatever you feel needs to be said.

4.) Don't include personal information. (Address, school you attended, city, zip code, etc) Your state is fine is you wanted to add that.
