Chapter 21

Levi

Once we get to the hospital we see Jacobs outside with other surgeons next to him. The paramedics unload John Doe and rush him inside.

"Good job, Levi scrub in, he's your patient now." Jacobs orders I nod my head enthusiastically and Jacobs starts jogging to catch up with the paramedics. Before I could follow him inside one of the paramedics' tugs on my shirt.

"Hey, this is his wallet." I grabbed the wallet and studied my patient, his name is Kyle Mince and behind his ID pokes out a red piece of paper.
I pull out the red paper and read in bold letters

AB- BLOOD TYPE 

CONTACT 555-555-555 FOR EMERGENCY DONOR

"Okay, thank you," I tell the paramedic then rush inside. I run to the front desk of the ER.

"Excuse me, nurse." The nurse typing on the computer looks up at me.

"Yes, Dr. Willson?" I waste no time handing her the wallet and the bright red card that resembles a blood-like color.

"Can you please call this number and tell her Kyle Mince was in an accident and needs immediate surgery. Have her come down here as soon as possible." The nurse looks at the red card and agrees. Now I have to find Jacobs and Kyle who are probably together on level 3 getting an MRI. I speed walk to the elevator and press the button. While I wait I take deep breaths trying to ease the adrenaline pumping inside me. The elevator dings as the doors open and people pile out. After the elevator was clear I step in pushing the button labeled 3 and patiently watch the doors close.

"Kyle will be okay, an Exploratory laparotomy doesn't need a major transfusion, he should be good if nothing bad happens. Then again there could be complications, there might be a major source of bleeding-" The elevator ding stops me from further worrying I step out and head to the MRI. Dr. Devon our Neurologists sits behind a computer looking at scans while Jacobs hovers over him. I clear my throat and both heads turn towards me.

"Ah, Wilson, come here and look at these scans." Jacobs gestures me to come closer, I do until I'm looking at the scans.

"Tell me what you see," Jacobs hushes not trying to break my concentration as Dr. Devon shows different sides of the scan.

"There's no bleeding," I comment.

"Yes, he has no head trauma." Dr. Devon announces a sigh of relief leaves my lips.

"Now let's get him into surgery." Jacobs pats my shoulder.

"Paramedics found his wallet, his blood type is AB- we've contacted his emergency contact who might be a donor" I inform Jacobs, he masks a thinking expression in response.

"Okay, we will still do the surgery and if the donor gets here we will take some blood for the patient's recovery." Jacobs crosses his arms in triumph. 

"Take Kyle down to the O.R and prep him I'll be there shortly." Once again Jacobs pats my shoulder and leaves. I take Kyle to the 4th floor and head to the O.R I grab my cap and put it on before entering.

By the time Jacobs arrives Kyle is ready, I scrub in and stand across from Jacobs as a Surgical Technologist stands by Jacobs side.

"Start the clock please," Jacobs announces to the room and an O.R. nurse starts the clock.
Jacobs makes a vertical upper midline incision and starts deepening the incision until he sees the liver. I suction all the blood that is covering the view of the liver. Once the liver is clearly visible we see a growing laceration.

"Wilson, do you think you can suture this liver tear?" Jacobs asks while packing around it. Nothing can go wrong it's an easy suture and there's not a lot of blood. I can do this.

"Yes, I can," Jacobs switches places with me and I hold the needle right above the laceration. I take a deep breath I begin suturing, I take my time not wanting to rush. Slowly the wound looks smaller everything's is fine and at that moment I'm confident. Out of nowhere, blood spurts out and the heart monitor is beeping loudly. Jacobs shouts and starts suctioning the blood.

"His right lung just collapsed," A nurse announces Jacobs is cursing out trying to keep the blood loss a minimum.

"He's not going to make it he's losing to much blood," Jacobs whispers and he isn't wrong we have no blood for him here.

"See if the donor is here!" I order a nurse who scurries away to the door.

"Wilson he's not going to make it he's losing to much blood." Jacobs try's to reason with me but I'm not having it. I haven't stopped closing this wound and I'm not about to, not when I'm so close.

"Give me a minute," I tell him I can hear the frown on Jacobs' face as I keep going. It only takes me two minutes to finish the laceration and for the bleeding to stop. Yet, it doesn't matter now, he's lost to much blood he's not going to make it. A nurse comes in with two pints of blood and we hook him up. There's no guarantee that he'll take the blood but we can only hope.

Half the nurses don't look as hopeful, they know failure is more common than success. Then again I have never had a patient die and I'm not going to let it happen now. We wait patiently to see if he rejects it, but he doesn't his body takes it willingly and everyone cheers. I pull my hands out of his body and Jacobs takes my spot making sure the suture will stay and not break. After he's finished checking we close him up.

"Okay we're going to flip Mr. Mince here and Dr. Fean will clear up all the fluids built up in his lungs. Then Mr. Meince will go into the ICU and make a full recovery." Everybody nods and cheers.

After the surgery, I head to the cafeteria and Jacobs visits me. He pulls a chair in front of me grabbing my attention.

"Hey Wilson, that surgery was really a miracle, I was positive he wasn't going to make. He shouldn't have survived, but he did. That was really brave of you. I can guarantee that no other surgeon would have done that. That is what's going to make you the best surgeon out there. Now, are you sure you want to stick with General? With your instincts, you can pick any field and they'll bow down to you."

"Thank you, Jacobs, but I really like General, it's my calling," I joke and Jacobs laughs.

"Alright, then kiddo see you around." I watch as Jacobs leaves the cafeteria and I'm left to eat alone.
Only 8 hours into my shift,
