"We need to get married" Derek repeats as Demetri hands me some Ritz crackers since I can't get enough of them "a child needs a mother and a father"

"And I have two other baby daddies who I have a great friendship and parenting relationship with" I swear, maybe I should have taken Dusty up on that offer to claim that the baby isn't Derek's "just be glad I told you, I didn't want to but after some serious debate the baby will decide what kind of man you'll be"

"What's that suppose to mean?" He demands

"Just like how it was phrased" Demetri hands me some water "at first I thought about marrying you but having kids with you? Nope. Just be lucky I don't believe in abortion for myself" I once did but after the one abortion, I regret that decision. Every now and then I think about it, before constantly but now? It comes and goes. I regret being bullied into thinking it was the right decision. I regret ever telling the father.

Just like now, I am regretting the decision to tell Derek "Madelyn" Derek pleads pulling out the ring, taking the ring before throwing it out the door when the door opened to the OB/GYN who is confused when I threw the ring over her head.

Demetri chuckles while Derek ran out after the ring, looking at Demetri mouthing "Lock the door"

Demetri quickly locked the door as the doctor greets "Dr. Grey, hello. I'm Dr. Lucy Fields I'll be your doctor throughout your pregnancy" she looks over to Demetri "and you must be the father..."

"No" Demetri shook his head no "just an emotional support friend, the father left the room to grab the engagement ring Maddie just threw out"

Dr. Fields looks my way "I don't like him and call me Maddie"

"Alright" she sits down on the stool "lay back for me please and scoot a bit forward" she turns on the machine and uses the wand thing that I've used thousands of times during transvaginal ultrasounds "so you wanna wait or"

"Go ahead" waving my hand and Demetri smirks shaking his head, Dr. Fields puts the wand inside of me and she moved the screen to show me. I smile seeing the sac before my smile falters when I see something "is that"

She lets out a surprised laugh "Twins, does it run in the family?"

"Yes" Demetri covered his mouth amused, Lucy pulls the wand out of me "oh wow..."

"Well" Dr. Fields stood up "enjoy your life Dr. Grey, while you still can"

She leaves the room as Derek rushed in "Did she just—was there an ultrasound?" He asks

"She has many patients" Demetri helped me off the table "needed to get going, no time to wait"

"How's the baby?" He asks "maybe we can do an ultrasound without her, your going into maternal-fetal medicine and"

My pager goes off along with Demetri's, we leave the room and when we are far enough I hiss "Did you know it was twins?"

"Didn't want to freak you out too badly" he says

Once in the ER "Mere?" She looks our way "Anything?"

"False alarm" she turns back to her charts "there was a papillary muscle rupture in the CCU, but the guy died before they could get him up to the OR. How was the ultrasound?"

"Twins" taking one of my charts to fill them out.

"Twins?" Both Alex and Meredith exclaim

"Twins" Demetri and I confirm

Alex whistles "When you go for something Maddie you go for it"

"Derek also wants to get married" I add

"Maybe you should marry him" Meredith suggests

"Are you crazy?" Demetri demands

Shaking her head no "Think about it. In the divorce since it's inevitable you can get sole custody, that land that Demetri has you can get since I've seen the pictures and you can build a pretty beautiful home there, and it'll get Derek to stop harassing you"

Tapping the pen I have on the counter top "You're not actually thinking about it are you?" Demetri demands

"My sister does make good points" thinking about it as Cristina appears demanding "Anything?"

"Guy died before he could be sent up to the OR" I say

"Crap!" She slaps the counter "I need surgeries and I need them now!"

"I'm going to marry Derek, right now" leaving the ER to find Derek, Meredith made some excellent points today. I mean after some time I can divorce him and during the custody battle, it can be proven how horrible of a man he is.

"You've got end-cirrhosis" it's a fancy way of saying end-stay liver failure "we can treat the symptoms but the only thing that can save your life right now is a new liver"

"How did it happen this fast?" Lexie demands

Lexie brought Thatcher into the hospital who was stumbling, disoriented and slurred his words. Meredith and I both hoped he was drunk, we still hope it now since if he is a candidate for a transplant then that means months of him and I don't intend on getting to know him.

"He bathed it in gin" Alex comments

"I think" shaking my head "it was scotch" I remember mom once telling me how Thatcher was the kind to buy scotch and drink anything with scotch in it.

"Alex! Maddie!" Lexie exclaims

"Lexie" Thatcher places a hand on Lexie's arm "your friend and Madelyn are right, but for the gin part? I was more of a scotch man. What's...what's the next step? Do I get on a list?" Looking over to Meredith noticing her dread, taking her hand she squeezes it.

"Um, Thatcher" The chief sadly says "the transplant board here has a rule, an alcoholic had to be sober for at least a year before they qualify for a donor liver"

"I'm only ninety days" Thatcher sighs

Lexie shook her head "I'll do it. I'll do it today, I'll go get tested right now"

"Lexie" Thatcher clears his throat "you"

"No" Lexie cuts him off "they just take a piece, my liver will regenerate"

"I can't ask you to do that" Thatcher moves to adjust in the bed "it's...it's major surgery"

"You're not asking" Lexie assured him "you're my dad. You want a kidney? I'll throw one of those in too" Meredith and I leave the room since this is too much.

"How was your wedding?" Cristina found me

"Fast" Derek and I were approved, too about twenty minutes. It happened. Derek is happy and wants to consummate the marriage, I told him I can't since I was bleeding and doctors orders are no sex until my next appointment which is in a month. But I never said what day or time "Biology is shit"

"What?" Cristina asks when we found her

"It's crap" Meredith complains as I eat some more crackers "utter crap. DNA. RNA. Doesn't make someone your family"

Cristina makes a face "Actually it does"

"He needs a liver transplant" I explain "and I can't be a candidate since I'm currently growing two human beings and Lexie is just offering up body parts and neither of us could stay"

"Wait" Cristina demands "your dad needs a transplant?"

Our pagers go off "Maybe not" Meredith whispers "maybe he's dead this time"

Meredith rushes off while I find Felix and ask "What would happen if Robert donated his liver?" I checked my phone and apparently Robert knows and wants to donate his liver.

Felix stops what he was doing to look my way "Given the age, Marcus would find out and Marcus would freak"

"But no Marcus" waving my hand "just Robert and given" looking around before mouthing "what he is..."

"Would be too risky" Felix pulls out his phone texting someone "he may be half you but his father's genes are stronger and it's not something anyone would risk—if Robert comes near the hospital Demetri will meet him and explain why he can't donate. So you found out..."

"A heads up would have been nice" a scowl is on my face

Felix chuckles "Apologies but the best heads up we could have given was you're pregnant, plus you're already under a lot of stress so..."

Tapping the counter, Felix picks me up and puts me on the counter "Thank you" smiling as I sit down here looking at him at eye level "feels better being eye level and not looking up hurting my neck. How come you won't introduce me to Marcus? We share a child together"

"Technically three counting the twins" Felix shook his head, rage fills his features "Marcus is one of the leaders but not the head leader, Aro is and he doesn't want you and Marcus to meet until after you finish your residency"

"Why?" I ask

"Marcus won't make the mistake he made before" good thing we aren't near anyone else "he'll change you the moment he sees you, given the children...newborns are dangerous and we can't risk anything. Especially with the children. Plus with the pregnancy?"

"Would he..." placing a hand on my belly

"He won't hurt the babies" he promises me "but the sight of you? It'll cloud his judgement and he won't hear the babies"

"And them" I ask "how will they feel?"

Felix gives me a look of sympathy "Jane and Alec love you. Most likely when Jane left it was because of Elia and hearing how you have two more kids, two other daughters. She was an only daughter for a long time, you know how you and Meredith reacted to Lexie? Especially Meredith?"

"So it wasn't just seeing me?" Shaking his head no, he stares at me "no matter how many times you say it wasn't my fault, I feel like it was. Maybe if I had been a better mom"

"Maddie" Felix sighs dramatically "you're the best mom ever, the twins love you so much. They are just...for them it's been a thousand years and having you back, it's a lot to take in for them and as immortals we have all the time in the world. I doubt they even realize what they did, that in the months it's been since meeting you that the twins probably think it happened yesterday not realizing it's been a much longer time"

"Do I or did I have any friends in the Volturi besides you and Demetri?" I ask

"Sulpacia, Athenadora" he nods his head "the other wives, many of the older members remember you and...you were very loved. You leaving" someone is nearby and back in code "it was very devastating, Demetri and I have a feeling that with you back now things will get better"

"Geez" letting out a laugh "no pressure"
