Into You Like A Train
Season 2 Episode 6
Margo rushes into the hallway near trauma one out of breath. "What do we got?"
Bailey gestures into the room and Margo peek her head in and gasps. "Holy hell!"
"His BP is looking steady at 90 over pal. She's had 2 hypotensive episodes to the low seventies." The paramedic informs the two. "We couldn't get the saw in with out moving them."
"They're never gonna fit into CT. We're gonna be flying blind. Get x-rays and labs and page me the minute you're done, please." Margo kindly asks Dr. Bailey.
"Excuse me!"
Turning around both doctors realize it's the woman stuck on the pill calling for them and they enter the room. "Hello, Bonnie. My name is Dr. Shepherd. I want you to try to not turn your head, we want you to try and not move as much as possible."
"Oh ok. ... So are you gonna pull this pole out of us anytime soon?"
"I'm sorry we can't until we get a better look on what's going on internally. But I assure you we will work as quickly as possible." Margo assures the both of them.
"Well, in that case, does anybody have a breath mint?" The woman jokes gesturing to the male on the other side of the poll.
"Uh O'Malley. Get them an X-ray." Bailey orders.
"Move them extremely carefully." Margo warns firmly.

Everyone stands in the X-Ray observation room examining the most recent scans as Derek walks in. He glanced at Meredith and gives Margo a quick kiss on the head.
"Hey." He notices Meredith with an IV in her arm, he sends a questioning glance to Margo and the woman. "What happened?"
"Uh, tequila." Meredith quickly answers refusing to make eye contact with the man.
"Look at this. These people are still alive?"
"Pole's tamponading the wound as far as we can see." Bailey
Informs the group.
"It's hitting the aorta."
"Is there anyway to operate without separating them?" George questions looking between all of the experienced surgeons.
"No." Derek immediately disagrees.
"What if we don't move the poll? What if we move one of the patients off the poll to get the saw in there? Then we can hold the pole steady in the other one. Move it very slowly and repair the damage as we go." Margo suggests, though it hurt her heart knowing that this meant only one could survive.
"Who? Which would you move?" George's face drops.
"With her aortic injuries, her chances of survival are extremely slim no matter what we do. But if we move her, we have a real chance of saving him. Unfortunately I don't think we have any other choice." Margo tells the intern.
"Well I could argue since her injuries are so extensive we should move him. Give her the best shot we can." Derek says.
"Whatever choice we make we have to make this call soon if we want our shot of saving either one of them." Margo tells the man.
"I'd like to examine them before I weigh in." Derek sighs as he looks over everything again.
"Of course, I trust your judgement. Take your time." She smiles at him then scrunches her nose. "Well... not too much time. A reasonable amount of time you know?"
He chuckles at her and shakes his head. "I'll page you when I'm ready."

"Mark?" Margo whispers as she looks over her shoulder in the hallway to make sure a certain neuro surgeon or neonatal Dr. are no where near. "You know I'm at work, I can't talk right now."
"I get that Derek's angry, alright?" Mark sighs. "But he shouldn't get to dictate who you speak to."
"You slept with his wife, Mark." She winces as the words leave her mouth. "Sorry, that was harsh."
"No, I get it." He assures but sounds crestfallen. "But that doesn't mean I should never get to see you and pumpkin again, Margo."
"You have a practice to run, Mark." She tries to come up with an excuse.
"I'll sell the practice." He exclaims. "I don't even know where you are because you haven't told me."
"I, uh." She stammers, just then she gets a page from Derek. "I can't, shoot I have to go I'm being paged. Look Addisons already here and Derek's stressed out enough."
"Wait!" He sounds angry now. And frankly he was. "Addison knows where you are?"
"I have to go!" Margo squeaks out before hanging up the phone in panic. "Crap, crap, crap!"

Now they all stood in front of the two patients looking seldom. "This is hard, because your body is in a certain amount of shock. It's preventing you from feeling pain. Feeling the extent of your injuries."
"Dr. Shepherd. We have a metal pole cutting a path through our insides. I don't know about Tom here, but I didn't expect to walk out of here anytime soon. So ...whatever it is you have to say just please just say it." Bonnie says looking to both Shepherds pleadingly. From the corner of her eye Margo can see George drop his head down.
"Okay Bonnie." Derek takes a deep breath. "In order to operate on Mr. Maynard, we have to separate you two. In order to do that, we have to move you backwards off the pole."
"Can't you just pull the pole out of both of us?" The male asks in confusion.
"If we did that, you would both start bleeding very quickly. Too quickly. Right now the pole is plugging the wounds. Once removed, the organs will shift and there's a great deal of damage." Margo informs the two, trying her best to keep her professional facade.
"So if you move me, I'll die?" The woman purses her lips in efforts to not cry.
"We're gonna do everything we can to save you both." Margo assures but she knows there's nothing more they can do for the woman at this point.
"No. No. If anyone body has to go it should be me. You just move me!" Tom states in desperation. He had spent all day living the most tragic day of his life with this stranger that become something more. They were bonded together by this.
"No. Mr. Maynard ... Mr. Maynard your injuries are less extensive. If we pull the pole from you as we operate around it, we have a better chance at repairing the damage." Margo shakes her head.
"Is my Danny here?" Bonnie questions hopefully, hoping to see her fiancé just one last time.
"There are delays at the airport, ah the storm." George whispers pitifully.
"Uh we could wait but ah the longer we do the higher the risk of infection for the..." Bailey offers.
The woman shakes her head. "No, no. This is better. Huh. Danny, he wouldn't understand. I've had a couple of hours to, you know, process all of this. But if, if he had to see me ... talk to me like this ... well, I just think it would be too hard."
Margo thinks of her own husband. What would've happen if he didn't die on impact. If they just had a few more minutes to say goodbye.

Margo rushes into the OR, Bonnie had started to crash as soon as they were both put under. "It's nobody's fault. We'll need to remove her now if we're going to have a chance. Team 1 continues, stabilize his body. Team 2 move her back and let's get the saw in there. This has to be fast and smooth, people. O'Malley you hold the pole in place. Whatever you do, don't pull on it, don't let it move. On the count of 3. 1, 2 ..."
They pull Bonnie off of the pole and gently place her onto another bed. "Let's start with her and we can see what we can do. Scalpel."
"Uh Grey. Get in here and help re-tract." Miranda orders.
"Her aorta is shredded. She's gonna bleed out." Margo sadly states as she takes a look of everything in front of her. Just then the monitor starts rapidly beeping.
"Got no rhythm."
Derek sighs and places all of his tools down. He walks toward the other patient, Tom. "Let's go."
They all leave the Bonnie's table except for Meredith who is still holding the retractor in Bonnie's chest.
"Everybody change gloves." Margo orders.
Meredith gapes at them all looking upset. "What about her? We can't just abandon her. We can't just abandon her!"
"I'm going in. Extend the sternum. Scalpel. We couldn't save Bonnie but we can save Tom." Margo sighs.
Meredith starts trying to revive Bonnie's heart by hand. George at the other surgery notices Meredith is still there as does Margo.
"Dr. Grey?" She calls out, not looking up from Tom's body.
"Lets go Dr. Grey." Miranda orders.
"What about her! We can not just abandon her! We have an obligation!" Meredith yells.
"Bailey go stop her." Margo orders.
"Meredith, come on. There was too much damage. There was never anything we could do. We have to let her go." Bailey soothes the girl as the monitor flat lines. "Time of death 3.49."

Derek enters the elevator to leave the hospital and notices Margo already waiting. As soon as the doors close she hits the stop button.
Derek is silent for a moment, not facing her, before he suddenly turns around and pulls her into a strong hug and cries.
"Oh, Derek." Margo sighs sadly, rubbing his back. "This isn't him."
