Some of you have voiced confusion on what exactly is going on so far in the story, so I have decided to go over the main points to hopefully get us all on the same page.

Akai's entire family (mother, father, twin brother) have passed away, and to escape being put in an orphanage Akai ran away from home. She bought wigs and contact lenses to avoid recognition and now lives a life of crime to survive.

She was captured by Kyoya's family, and they made her go to school since Kyoya has been assigned temporary responsibility for her until her identity is confirmed.

At school she was signed up by the name of Loretta since she lied to Kyoya and told him that was her name. At Ouran everyone first mistook her for a boy, so she decided to pretend to be a boy and go by the name Loret. Only her teachers and Kyoya know her true gender. (Plus everyone thinking she's a boy helps with getting her into the host club haha).
