To be a good surgeon, you have to think like a surgeon. Emotions are messy. Tuck them neatly away and step into a clean, sterile room where the procedure is simple. Cut, suture and close. But sometimes, you're faced with a cut that won't heal. A cut that rips its stitches wide open.

Derek, Mackenzie and Addison were in the lobby of the hospital.
Derek looked annoyed with Addison. "Addison, what are you doing here?"
Addison ignored his question. "Your hair's different."
Derek crossed his arms. "A lot of things are different."
Addison smiled. "It's longer. I like it. It's very Russell Crowe." She tried to run her fingers through his hair but he moved back.
Derek sighed. "What are you doing here?"
"What are you doing here? You just pick up and leave everything? Your house, your practice, your friends? You had a life in Manhattan. Mackenzie had a life in Manhattan." Addison shot back.
"Had." Derek emphasized.
Addison nodded. "And now you have a girlfriend in Seattle. She seems sweet."
Derek glared. "The ice you're on. Thin."
Addison continued. "She's young. That whole wide-eyed, ooh he's-a-brain-surgeon thing happening, but still sweet. Which was what you were going for, right? The anti-Addison?"
Derek gave her a cold look. "If you came out here to try and win me back, forget about it."
"I did. I flew all the way across the country to reminisce over wedding photos, get drunk, fall into bed, and make you realize you can't live without me. I also flew all the way across the country to try and make things better with Mackenzie. Relax. Derek, I'm here for work. I'm helping the TTTS case you guys admitted last week and from Richard's briefing, I should be — " Addison was cut off by her husband.
"Richard, he knew you were coming out here?"
Addison nodded. "He asked me to come. Didn't he tell you?"
Derek shook his head. "No. He didn't."
Addison nodded. "Hmm. Surprise. The hair, though. You know I've always had a thing for Russell Crowe."
Mackenzie just sat in a chair, knowing that Addison coming back into her life was going to be a disaster.

Meredith, Cristina and George were all sat at Joe's bar.
"Let's play a game of whose life sucks the most. I'll win. I always win." Meredith said.
Cristina shook her head. "No, you don't want to play with me."
Meredith looked at Cristina. "Oh, I do. I'll even go first. Derek's married. His wife is Mackenzie's stepmother."
George spat his beer out.
Cristina looked over at George. "George, beer is dripping from your nostrils."
Meredith shrugged. "Told you I'd win."
Cristina shook her head. "No, you didn't win."
"Did you hear me? I said Derek is married. As in pigheaded, adulterous, liar married. Nothing you could say could top that." Meredith said.
"I'm pregnant. I win."

The Twisted Sisters walked up to a desk where George and Izzie were standing.
Izzie looked at Meredith. "Why are you back here tonight, don't you have a date with McDreamy?"
George grumbled. "More like McMarried."
"McWhat?" Izzie asked.
Meredith shrugged. "I came to check on Joe."
George looked concerned about the bartender. "You think he's gonna be ok?"
Cristina sighed. "You think he's gonna need an operation?"
Derek walked up. He was greeted by cold glances by the interns.
"Operation, yes. Ok, hard to tell. Basilar artery's blown up like a balloon. Subarachnoid bleeding. Aneurysm the size of a golf ball."
"No way to clip something like that." George commented.
Cristina nodded. "Not without magic fingers."
"Or a standstill operation." Derek suggested.
Cristina's eyes widened. "You're doing a standstill? He's doing a standstill operation."
Derek nodded. "I'd like to try. First I need some additional patient history, overnight labs, and a cerebral angio."
He tried to hand the chart to Meredith.
"I'm drunk." Meredith stated.
Derek sighed. "Meredith."
Meredith walked away and George grabbed the chart. Derek tried to follow Meredith but the other interns step in his way.
Izzie glared at Derek. "McBastard."
Derek left. Cristina toon the chart from George.
Izzie cleared her throat. "Ahem."
George gave Cristina a deadly look. "What are you doing?"
"Uh, I am on her side, but we're talking a possible standstill here. Recognize."

Meredith was walking out the front door as Derek caught up to her.
"Meredith!"
Meredith growled. "Go away!"
Derek ran to her. "Just wait. We should discuss this."
"Here's a thought. No! Quit following me!" Meredith said loudly.
"At least let me explain." Derek tried.
Meredith looked murderous. "Explain? You know when you should have explained? The night we met in the bar. Before any of the rest of it. Yeah, that would have been a good time to discuss it."
Derek gave Meredith a look of empathy. "Look, I know how you feel."
Meredith was angry. "Do you? Somehow I doubt that. Because if you did, you would shut up, and you would turn around and go back inside, because you would realize that I am this close to getting in my car and running you down in the parking lot!"
Meredith left, and George ran up to her with an umbrella. Derek went back inside.
George looked at Meredith. "Where are your keys?"
"I'm fine!" Meredith countered.
George shook his head. "Meredith, give me your keys. Let's go home."
"Take me with you. I'm not going anywhere near the trailer tonight." Mackenzie begged.
Meredith nodded. "Come on, Mack. You can spend the night tonight."

The next morning, Derek walked into Richard's office. Burke was the interim chief of surgery.
"So. Congratulations are in order." Derek said.
Burke laughed. "Don't sweat it Shepherd. I'll only be your boss for a few days."
Derek nodded. "I'm well aware of Richard's recovery time. I'm the one who operated on him, remember?"
Burke also nodded. "I do. You operated. He survived and chose me to take over while recruiting your wife."
"Clearly, he has brain damage." Derek remarked.
"Or is it your ex-wife? I'm a little fuzzy on that." Burke teased.
Derek gave Burke a look. "We're separated."
Addison walked in. "Sorry to interrupt, Dr. Burke."
Burke smiled at Addison. "You're never interrupting."
Derek glared. "She's always interrupting."
Addison looked at Burke. "I was just checking to see if Dr. Burke secured the intern."
Meredith walked up. "I'm the intern you requested?" She asked Derek.
Derek shook his head. "He did." He said, pointing to Burke.
Derek looked at Addison then Burke, Burke smiled and walked away. Derek walked away and left Meredith and Addison there staring at each other.
Talk about awkward.

Burke walked up to Bailey, who was sitting in an office. "Got a second?"
Bailey looked up briefly. "Depends on what you need."
"Who the hell is Dr. McDreamy?"
Bailey looked to where Derek was talking to Addison and George passed by.
"Me. I'm Dr. McDreamy. I'm tall, handsome. I like to lean against things and ponder the difficulties of dating beautiful women. I'm trying to be a surgeon here!" Bailey said.
Burke walked up to where Derek and Addison were talking.
Derek looked at Addison dangerously. "That took a lot of nerve."
Addison rolled her eyes. "Oh come on, she came highly recommended."
Derek pulled her away as Burke drew near. "Right."
Addison raised an eyebrow. "So you don't recommend her?"
Derek sighed. "No, I did not say that."
Addison smirked. "Just not for her medical skills."
Derek groaned. "Oh, would you shut up?"
Meredith walked up. "Dr. Shepherd."
Addison and Derek both answer Meredith. "Yes?"
Meredith cleared her throat. "Labs confirm what look like abnormalities on the ultrasound. I think you should come and see for yourself."
Addison nodded "Fine. Let's go."
Derek tried to get Meredith's attention. "Meredith. Meredith."
Meredith shook her head. "Don't."
Burke was looking on from near the surgical board. As Derek noticed him, he turned to leave, almost running into George.

Mackenzie felt George staring at her while she ate her lunch. The two were sitting on gurneys in the basement corridor.
"Are you going to just stare at me, or are you going to say something, George?"
George broke out of his stare. "Why didn't you say anything about your papa being married? Meredith deserved to know."
Mackenzie sighed. "Because Addison's my stepmom, George. I don't have to share every private detail of my life with Meredith, no matter how much I like her. Besides... why would I want to talk about Addison? She's the reason we moved to Seattle from Manhattan in the first place."
"Wait... slow down, Ellie. Tell me everything. I won't judge you, I swear." George told her.
Mackenzie looked at her brother. "I had just gotten home from a trip to the mall with Addison and my dad, Mark. We all had dinner together and watched a movie before Dad put me to bed. Papa came home late that night. He said that he went upstairs, checked on me and kissed my head before he went to his bedroom he shared with Addison. He stepped on a jacket. It wasn't his jacket; it was my dad's jacket. That's when Papa opened the bedroom door and saw Addison cheating on him with my dad. I woke up at 3 am that night — I was in my father's arms while we were in a hotel room. He was crying, and when I asked what was wrong... he told me that Addison cheated on him with Dad. That was the first time I had ever seen my papa cry, and I just hated seeing how broken he looked. His whole world fell apart, George. He lost his wife and his best friend that night. My family was torn apart. It was the worst day of my life."
George pulled Mackenzie into his arms, rubbing her back as she let out heartbroken cries and sobs.
"It's okay, sweetheart. Georgie's here; Georgie's got you. I'm so sorry, Ellie. I'm so sorry you had to go through that. Is that why you've avoided Dr. Montgomery-Shepherd all day? Because her being here reminds you of what she and your dad did?"
Mackenzie just nodded and buried her face in the crook of George's neck. "Mm-hmm."
George consoled the crying girl, kissing her head and doing everything he possibly could to make her feel at ease.
His poor baby sister shouldn't have gone through what she did. But Derek shouldn't have kept his marriage to Addison a secret from Meredith, either.
The real victim here was Mackenzie.

Addison and Meredith were in Julie's room. Julie was the TTTS patient, and she was out of her surgery.
Addison smiled. "See? Just a small scar."
Julie looked impressed. "And my babies?"
Addison smiled. "Your babies are doing very well. And Dr. Grey will be back to check on you a little bit later."
Julie's smile faltered. "Actually, I'd prefer it if Dr. Grey were taken off the case."
Addison raised an eyebrow. "Why, is there a problem?"
"Just reminds me of someone I don't like very much. Someone my husband likes a lot. Particularly in lingerie. You understand." Julie said.
Addison shook her head. "No, no, I don't understand."
Julie looked at her doctor. "Well, she's sleeping with your husband, right?"
Addison looked at Julie. "Ms. Philips, I lack Dr. Grey's class and patience so, let me set the record straight. My husband didn't cheat on me, I cheated on him. So the wronged woman here, Dr. Grey. So, I think you owe here one hell of an apology."

Meredith and Derek were sitting on the porch of his trailer. Mackenzie was asleep in her bed.
Derek sighed as he told Meredith about why he left Addison.
"One night I parked my car, I unlock my front door, go inside my house, and something's different. Nothing's different, everything's the same, but yet, still, something's different. And I stand there for a while. And then I know. See, there are moments for me, you know, usually when I'm in the OR, when I just know what's gonna happen next. So I go upstairs. I go check on my daughter, who's fast asleep, and I give her a kiss on the head. As I close the door to her room and I'm walking down the hall, I'm trying to prepare myself for what I'm gonna see when I go into my bedroom. I step on a man's jacket that doesn't belong to me. And everything I think I know just shifts. Because the jacket that doesn't belong to me is a jacket that I recognize. And what I know now is that when I go into my bedroom, I'm not just gonna see that my wife is cheating on me. I'm gonna see that my wife is cheating on me with Mark, who happened to be my best friend and Mackenzie's father. It's just so pedestrian, common and dirty, and cruel. Mostly just cruel. What was even worse was that they were having sex in my house with my daughter down the hall. So I took Mackenzie with me to a hotel for the night. I left, took my daughter with me and came out here."
Meredith nodded. "And you met me."
"And I met you." Derek agreed.
Meredith sighed. "Well, what was I to you? The girl you screwed to get over being screwed?"
Derek shook his head. "You were like coming up for fresh air. It's like I was drowning and you saved me. That's all I know."
Meredith sighed sadly. "It's not enough."
Meredith got into her car and left Derek's trailer.
Derek was pacing the porch of his trailer, drinking his beer and looking upset. Derek went inside the trailer and paced some more. He checked on his sleeping daughter before he went back to the porch, slamming the door.
As Derek sat down, he ran a hand through his hair.
Everything was a mess now.

The say practice makes perfect. Theory is, the more you think like a surgeon, the more you become one. The better you get at remaining neutral, clinical. Cut, suture, close. And the harder it becomes to turn it off to stop thinking like a surgeon. And remember what it means to think like a human being.

Next Update: Enough Is Enough (No More Tears)
*I have officially started writing season 2 of Grey's Anatomy. There will be a lot of drama between Derek and Addison, as well as Addison and Mackenzie.*
