Separated or last.

Separate:
To be severed from.

My heart lacks a decision:
Lost, separated, severed.
My mind is broken in two:
Split, severed, cut, injured.

My hand tries to finish the poem:
Complete, end, leave.

Every letter put in is used. Every word has meaning.

Finish:
Complete something.

Leaving something.

Leave:
Going away.

A poem can't be finished.

It can't be ended.

It can't be completed.

My hand wavers,
Desperation causing it to shake.

Finish the poem.

But my hand doesn't write.

It can't be finished.

It can only stop moving.

I stop writing,
Stopping the poem from moving
And leaving it behind.

My mind wishes to end it,
But also to keep on going.
