SOME TEMPLATES:

I usually use PicsArt. So open the app and a select a picture containing a face.

Chose any cracked image. Some of them are given above. I usually prefer black and white. In this case I have chose this:

Now, decrease it's opacity and use overlay according to the picture.

And your image is ready.
