How to make the OC form, I will show you by example!

Name (first): "Maya"
Name (middle if possible): "only mommy and daddy can know that for when I'm In trouble!" Phoenix
Name (last): "Dragneel."
Age:  "13"
Magic: fire dragon slayer.
Family: "mommy and daddy of course!" Natsu and Lucy are her parents.
Personality: "find out silly!!" bright, bubbly, cheerful, playful, fun loving, childish.
Flaws (yes they need to be there): "not telling" does get annoyed and really touchy on a lot of things.
Gender: "i'm a turtle" female.
Looks: "look at me!", Long pink curly hair down to her shoulders (even in the pigtails) in high pigtails held up in a bow, green eyes and has teal blue glasses for being near sighted and mostly blind in one eye. A pink tank top with a white mini skirt and stalkings that have kitties on them, and even pink converse shoes, and obviously red lip gloss and purple eye shadow with teal blue eyeliner.
Other: "uh... I'm allergic to Bees! If i get stung by one I'll stop breathing! And I'm scared of shots! Scary!!!"
