“Paging Doctor Sharma, please report at the emergency room immediately. I repeat, Dr Nandini Sharma please report at the emergency room immediately.”
As soon as I heard the announcement, I was on my feet dashing towards the emergency room on the ground floor. I was all set to leave for the day, my 16 hour duty was completed and my bed was calling me. Just before I could fling my handbag on my shoulder and call it a day, the speaker in our lounge made the announcement. There was total chaos in the emergency room. I reached my assistant who was already present there and waiting for me.
“Dr Yash what happened?” I asked, taking in the scene.
“Dr Nandini there was a major accident between a bus and a car. It seems the driver of the bus was drunk and lost control of his vehicle that dashed into the car coming from the opposite side. Luckily the bus was empty with just the driver in it. He is not much injured, just a few broken bones is what we are expecting.” He took me near the patient who was withering in pain. Dr Desai and his team were examining him.
Yash gestured to move ahead at the next bed where a man laid unconscious, his face covered in blood and small glass pieces. A team of junior doctors were cleaning his face which was beyond recognition.
“This guy is our concern. He has a severe TBI. He was driving at a very high speed so the impact was severe. His head got banged on the dashboard and he was thrown out through the windshield where again his head was banged with the truck. He needs an immediate diagnosis of the skull. He was breathing but was unresponsive and was bleeding profusely on arrival.”
“Okay let's get his head scanned immediately and get me the reports ASAP. Also prepare the operation theater for the surgery.” I instructed him
I went through some case studies where there is a major TBI (Traumatic Brain Injury). As we are not sure about the condition of the patient, I can't do the final process of treatments. After an hour and half there was a knock on the door and Dr. Yash came in. From the expression of his face, I knew we were dealing with something serious.
“Reports are not good.” He said as soon as he sat and passed me the scans. I clipped them on the light screen.
“CT scan showed severe left frontal contusion with an epidural hematoma, a right frontal contusion and right temporal contusion, extensive traumatic subarachnoid hemorrhage, left-sided mass effect with shift of the midline structures to the right, and a basilar skull fracture extending through the right occipital bone.” He concluded .
“On an immediate basis we need to do the left frontal craniotomy with evacuation of epidural and subdural hematomas with resection of the left frontal contusion.” I told him
“Yes, I thought the same.” He replied
“Let's get going then. We need to discuss this with Dr Awasthi” I got up from my chair collecting all the reports. Dr Awasthi is the Head of our department.
“He said you can go ahead with your treatment. He trusts you and the patient is already in OT.” Yash told me while opening the door of my cabin for me. I huffed.
“What about his family? Are they informed?” I asked walking towards the OT
“The police authorities are still searching for his identity. He didn't have any documents with him.” I stopped in the middle listening to his reply.
“Who will fill the consent form then?” Was my genuine concern
“Umm…” Yash gave a sheepish smile scratch his head
“Dr Awasthi right” I concluded
“Hmm.” Yash just hummed as we resumed our walk.
We went to the OT and performed all the pre-surgery activities. Then we approached the surgery table. The whole team was ready. I had already instructed them about all the proceedings.
But as my eyes fell on the face of the patient, I was petrified. My head started spinning and I was about to lose my consciousness.
