In this world familiars appear when a kid turns 5. If the human is hurt the familiar feels the pain and vice versa. If one gets sick so does the other.

Scenario:
You and your familiar are tracking Julian down, somebody had offered to pay you a lot to kill him.

You see him, curled up in a blanket by a fire, Avalon sitting near him. Both looked extremely tired and sick, they had gotten sick from traveling to avoid assassins, neither of them notice you as Julian leans on Avalon, coughing, you had medicine with you in case you has gotten sick yourself.

Literally the first scenario but the roles are reversed, I'd prefer if the other is done as well but meh
