I don't require some format but if you really need one then here is the default I use in my files:

Identification - [Their name along with any prefixes such as Sir, Lady, General, ETC]

Height - [Preferred in CM but FT'IN" is allowed]

Weight - [Preferred in Kg, but LBS is allowed]

Ethnicity - [Where ethnic group are they apart of? Mostly just to help imagine looks.]

Nationality - [What country do they live in?]

Spoken Languages: [What tongues can they speak?]

Religion - [Religion]

Sexuality - [What are they looking for? Helps with scenarios or jokes.]

Birth Place - [Where are they from?]

Current Residence - [Where do they live?]

Family - [Got any?]

Occupation(s) - [Got a job?

Appearance -

[Description of their appearance to fully finish their mental picture.]

Background -

[Got a history?]

Self Introduction -

[How would they introduce themself?]
