Tweaked out over this for a week (Body Horror warning ig)
Here's the Nepal Headcanon!!!
Lore:
Nepal tries to ignore the mouth, and can usually do a decent enough job at it. However, when an intrusive or fleeting thought, or the irrational urge to say something that probably shouldn't be said (all normal things to experience, btw) arises, the mouth will yell what he would rather have kept quiet about.
Yk when you're really angry and have the urge to eat drywall or throw a baby?
Nepal gets that too (it's normal!) but can't stop himself from actually following through. Cute aggression is another one that scares him, for fear of actually hurting the cute thing.
It's very difficult to concentrate when the mouth is chewing on the fabric of his hoodie constantly, and whispering things that he would like to rather not hear.
He tries to believe that he's in control.
But who is the real Nepal?
Characteristics:
Nepal's and the mouth's jaw are connected. When the big mouth is closed, his jaw functions normally. However, when the big mouth opens, its jaw swings down, and Nepal's swings to the left and down. Hinge-on-hinge type stuff. While the motion of the jaw isn't painful, his skin stretching can be (assuming the mouth opens wide enough). Any pain that is felt by the mouth is also felt by Nepal.
Nepal has two chins—one for himself, and one for the big mouth. The big mouth's jawline also shows on the left of Nepal's face. The skin is stretched tight over his chin and throat.
Nepal also has two voiceboxes!
Apart from that, Nepal's body is a normal humanoid, although when the big mouth gets violent, Nepal may lose control over it.
Real-life-applicable lore (dunno what else to call it):
Nepal has severe impulse control disorder, represented as a talkative and unwelcome parasitic, sentient being that he can't get rid of. It talks out of turn and says things that were better left unspoken, and occasionally gets violent. It also can see—meaning that the being can consume unwanted pieces of information, and all Nepal can do is live with it.
Impulse control issues are common in young children. Usually, they grow out of it. Nepal didn't.
ICDs (Impulse Control Disorders) are characterized by a person having an inability to control their impulses, or urges. (Very rarely do they entail violence—by adding that detail to this headcanon, I am not trying to insinuate that people with ICDs are uncontrollably violent. Only in severe cases is violence truly involved.) After a person indulges in the temptation, he or she may feel gratified and then guilty for doing so (five behavioral stages: impulse, resisting, acting, relief, and guilt. The final stage doesn't always occur).
ICDs may manifest as:
Sexual Compulsion—the individual may have in increase in sexual behavior and thoughts. This can lead to depression, increased risk of STIs, unwanted pregnancy, and questionable choice of partners. Hypersexuality and Sexual Compulsion can be considered synonymous despite the American Psychiatric Association rejecting this.
Internet addiction—the individual may spend excessive time engaging in damaging activities online, such as gambling, over shopping, or consuming pornography. No studies have been conducted yet of its prevalence.
Compulsive shopping—typical of females in the late teens to early twenties, the individual may buy a lot of items, even when they are unneeded, unnecessary, or unaffordable. This is usually considered chronic.
Pyromania—the individual may have the urge to just start a fire for the sake of gratification. This does not have to be sexual.
Intermittent Explosive Disorder—a subset of ICD, this is characterized by recurrent aggression that is much more than needed, given the stressor.
Kleptomania—the urge to steal, entirely for gratification. This does not have to be sexual.
Late Parkinson's disease may also manifest as impulse control issues.

This is not mean to romanticize the condition; rather, it is meant to bring attention to it and allow me to
1) stimulate the braincell and do some research bc mental disorders are interesting
2) allow me to make connections between different and seemingly unrelated fields (mental disorders and art and flags, anyone? What about the human body vs an industrial plant??)
3) create a metaphor and project it onto a person (or a country ig) to allow me to better understand and empathize with those who have the condition(s). It's honestly a very fun and informative thing to do. I highly recommend it.
